Addition of New Drugs to the Formulary

Application for new medicines to be included in the Formulary may be made at any time, by a consultant holding a contract with SMUHT. The procedure is as follows:

  1. You should make a written request/email for the medicine to be added to the Formulary. The letter should be sent to:

    Mrs Sue Banfield
    Principal Pharmacist
    Medicines Information
    3rd Floor Tower Block
    Wythenshawe Hospital
     
    This letter should outline the need for the new item.

  1. You will be sent an 'Application for Use of a New Medicine' form, by email wherever possible. This will include a request for details of evidence for the superiority of the new item against existing Formulary medicines and a declaration of interest. It is helpful if any relevant clinical data is included in support of the application.

  2. You should return the application form to Mrs Sue Banfield, preferably electronically followed by a signed copy by post.

  3. A review of the medicine will accompany the application when approval from the Medicines Management Committee is sought. Following approval by the committee, divisional approval will be sought.

  4. After the meeting, the Chairman of the MMC will advise you in writing of the Committee's decision. If approved, the medicine will be included in the Formulary.

  5. Medicines should not be referred to GPs for prescribing unless they have been approved by either the MMC or the Manchester Prescribing Strategy Group has approved them.
