Risperidone/Olanzapine
In March 2004 the CSM recommended that risperidone and olanzapine should not be used for the behavioural symptoms of dementia or for the treatment of psychosis where there is a history of stroke/TIA or risk factors for cerebrovascular events. This is due to an increased risk of stroke and TIAs (for every 6 patients with dementia treated, one extra cerebrovascular event could be expected to occur). It is possible that this risk extends to all atypical antipsychotics but current recommendations do not contra-indicate use of quetapine, however all atypicals should be avoided.
For patients admitted on these agents, guidance is available at
www.rcgp.org.uk/corporate/position/drugs.asp
