The recommendations in this formulary do not cover all clinical circumstances. For further advice contact microbiology (see contact details below).
This formulary does not cover paediatrics, SCBU, GU medicine, cardiothoracic intensive care or the acute intensive care unit - separate antimicrobial policies are available for these areas.
General principles of antibiotic prescribing
Always take appropriate samples for
culture before starting therapy - but do NOT delay appropriate
meningitis therapy
Always enquire
about previous drug reactions and allergies
Consider patients
renal and hepatic function before prescribing
Ensure that the
most appropriate agent and dose is chosen for your patient
Document the Route, Indication
and Duration
(RID) of the treatment course when prescribing antibiotics
Intravenous
antibiotics should be reviewed on a daily basis
